Persivia's comprehensive solutions standardized data across the network, integrating tools like Care Coordination, Risk Stratification, and Cost Analytics. With the integration of multiple EHRs managing 2 Quality and 7 VBC programs, Prime Healthcare achieved notable outcomes, including a 14.6% decrease in readmission rates and $17MM in incentive payments.
